Weekend Senior Structure:

Saturday and Sunday mornings are staffed similar to our weekday structure, with each team having an assigned senior (ie: AT1 Senior, AT2 Senior, PA Senior, GI Senior, and NeuroEndo Senior) which will be pre-assigned and appear on Amion.  These roles will start at 6am and end at 6pm on Saturdays and Sundays.   We make all attempts to preserve your same weekday senior role during your weekend call shifts.  With this structure, it is an expectation that sign-outs should be up to date by the time you leave on Friday evening, so that the weekend senior has accurate information on their patients.

GI Senior/Bed Czar:  The GI Senior will function as Bed Czar during the weekend days.  The expectations of the GI Senior/Bed Czar are to:

  • Round with the GI Team
  • From 6am-6pm, receive ALL phone calls from the Emergency Room for Hospitalist Admissions
  • Triage these admissions (via the HAR Admission Tracker) to the appropriate daytime team based on census and acuity (ie: PHAST vs. Academic Team)
  • Listen to the patient's story from the ED:  if unclear whether PICU vs. Floor, SEE patient in ED to assess safe dispo.  If elements of the story are unclear, see the patient in the ED for additional history
  • Assign an admission team
  • Place orders (including appropriate virtual pager information)
  • Notify appropriate team senior of the admission to their team
  • From 6am-11am:  the expectation is that the Bed Czar will perform the full admission, as this is protected rounding time for the busy academic teams.  Please place all orders, see the patient, and write the H&P during these hours.  After rounds, discuss this patient with the appropriate team in order for them to take over care of that patient.
  • This role is analogous to the Hospitalist Bed Czar during the weekdays
  • If the hospitalist teams are busy, help with the admissions after rounds

At 6 pm on Saturdays and Sundays, the 5 daytime seniors will transition to night roles. We suggest a "team huddle" at 6pm for any transition that occurs between GI senior and AT senior.

As a weekend senior, the expectation is that you function just as you would during the weekday.  Take ownership over your team!  Help your interns, facilitate admissions and discharges, and perform all ICU transfers.  Help with updating families if your interns are busy.

As always, we would love your feedback on these new roles and structures.  These exist to help all of our residents feel more supported, especially on nights and weekends, and we are very open to feedback on how to improve these roles throughout the year.
