Welcome to the Neurology/Endocrine Service!

Your 4 week rotation on Neuro/Endo will be an incredibly rewarding experience in an inpatient subspecialty pediatrics. Here are some details to help you learn more about the rotation:

Rotation Structure: The Neuro/Endo service is composed of two attending physicians (Neurology and Endocrinology), two fellows (Neurology and Endocrinology), a senior resident (PL-3), and 3 interns.

Rotation Contacts: You will have a tremendous amount of support throughout this rotation. Your attending physician, fellow, and senior resident will be great resources. For additional questions, please contact Andrew Abreo (chief resident), Dr. Marc DiSabella (Neurology rotation director), or Dr. Priya Vaidyanathan (Endocrine rotation director).

Prior to Rotation: It is important to review the following material prior to the first day of your rotation. The text below is linked to additional information.

Teaching:

Neuro/Endo Teaching Schedule

Rounds: Neurology rounds begin at 9 AM every day. Endocrine will check-in with the senior resident/intern from 8:15 - 8:30 AM daily with the exception of Wednesday. The check-in will be in person with the service attending and also NP/Fellow as applicable. Endocrine Family Centered Rounds will take place from 11:30 AM - 12 PM if there is afternoon Endocrine clinic or 1 - 1:30 PM if there is morning Endocrine clinic.

Feedback: Feedback is a key part of the rotation! Feedback contributes to growth and development as a learner and mentor. It can come in various forms from on-the-fly to Feedback Friday.

1. Structured Clinical Observations (SCO's) are required for your annual program portfolio and serve as a way to receive verbal/written feedback. Seniors, fellows, and attending should observe a portion of a patient encounter and provide this feedback weekly. Notify your attending if there is a particular encounter you would like feedback on.

2. Each intern should complete one (1) fellow observed Neurology exam per rotation.

Sign Out: Intern sign out (handoff) must start at 5 PM Monday-Thursday and at 6 PM on Friday to facilitate patient care and compliance with duty hours. Similar to other rotations, the academic teams use I-PASS to ensure safe hand off.

1. Neurology discharge huddle will start no later than 3:30 PM and be led by the attending and senior resident. The huddle should focus on discharge needs while incorporating teaching.

2. The Neurology ward attending will observe I-PASS handoff one time per service and use the I-PASS feedback tool give give specific feedback.

3. Review the I-PASS sign out tool.

Discharge Education Initiative: Discharges are often complex and require attention to detail to ensure a smooth process.

1. Neurology Discharge Huddle will address the following:

  • Teaching: Discuss why testing (i.e. EEG, LP, MRI) resulted in specific management and why patients were placed on specific medications or why not medications were indicated.
  • Discharge: Review discharge appointments (date, time, and clinic attending/fellow), discharge medications/equipment, and barriers to discharge (i.e. patient still on Oxygen, no transportation, waiting on MRI to be finished, etc.). Derrick will try to be present on discharge rounds.

2. On Rounds, please restate all medications for every patient to confirm the following:

  • Drug name
  • Form (liquid and concentration if applicable)
  • Volume and times per day
  • Refills on discharge
